Credit Application

Once you’ve established a new account, potential resellers need to fill out a copy of our credit application. Based on how you’d like your account set up with ARLINGTON, please provide the following information:

For Net-30 Terms

Complete the top half of the credit form, provide 3-4 trade references within our industry, a bank reference, and have the owner or officer of the company sign the bottom of the application. Fax or email the completed application to your Sales Representative, with a copy of your resale certificate.

For COD

Complete the top half of the credit application form and provide a bank reference. Have the owner or officer of the company sign the bottom of the application. Fax or email the completed application to your Sales Representative, with a copy of your resale certificate.

For Credit Card

Complete the top half of the credit application form and have the owner or officer of the company sign the bottom of the application. Write “Credit Card Account” in the middle of the form. Fax or email the completed application to your Sales Representative, with a copy of your resale certificate.
